Request To Cancel Monthly Pass Agreement:
Cancellation request forms may be submitted at any time during the month (prior to the next recurring payment date). After submitting a cancellation request, passholders will have access to the center and amenities for the remainder of the month.
Pass cancellations may not be submitted two or more months in advance.
Passholders must hold the monthly pass for a minimum of two months before it can be canceled. This two-month period begins on the day the pass was purchased.
By checking this box, I am providing written notification to cancel my monthly recreation center pass agreement at the South Suburban Park and Recreation District (SSPRD). I understand my cancellation form submitted today will discontinue recurring payments starting next month. I understand that I cannot make a request two months or more in advance for cancellation. I also understand a two-month minimum commitment is required. If the two-month period has not yet been met, my monthly pass can be cancelled once it has been fulfilled.
